Information leaflet and request for data processing authorization
(Law Decree 30/06/2003, no. 196)

Pursuant to and by effect of art. 13, Law Decree 196/2003 - Law on the protection of personal data, Cisaplast S.p.A., based in Via POLISCHI, 3 -  SUZZARA, MN, as “Controller” (person responsible for taking decisions as regards purposes and methods of personal data processing and instruments used, including security aspects), informs as follows:

 

Processing methods and purposes

The personal data provided by you as website visitor and potential customer will be processed according to applicable legal provisions, using manual, IT and computer instruments, with logics strictly correlated to the purposes themselves and such processing could involve all the operations indicated in Article 4, sub-section 1, letter a), of Law Decree no. 196/2003. Providing data is optional, nevertheless failure to provide processing authorization (art. 23 “Consensus”) could prevent the carrying out of normal business activities. The provision of data also represents a necessary requirement for the correct fulfilment of tax obligations, which are mandatory by law. The data will be organized and filed in electronic and/or paper archives and will be processed using adequate instruments suitable for ensuring security and utmost confidentiality.

Providing data

Without prejudice to your entitlement to provide data or not, kindly be informed that failure to provide such data will result in its being impossible for Cisaplast S.p.A. to consider any service requests from you.

Sensitive data

Please do not indicate any sensitive data (likely to reveal racial and ethnic origin, religious beliefs, philosophical beliefs or any other beliefs, political opinions, membership of political parties, unions, associations or organizations of a religious, philosophical, political, union nature, or personal data revealing state of health and sex life). Any sensitive data indicated will be immediately cancelled in the absence of a written statement from you – following the careful reading of this information leaflet – authorizing their processing by Cisaplast S.p.A.

Parties to whom the data can be notified and scope of disclosure

Without prejudice to the notices, disclosures made in compliance with the Law, all the data collected and processed may be notified and disclosed to the employees or associates of our Company and also to third parties. Any notification and disclosure of the data, howsoever processed, shall be related to the carrying out of the business activity of our Company. The data may also be notified and transferred abroad, in accordance with Legal provisions (art. 25 “No notification or disclosure”).

Processing controller and manager

Data processing controller is Cisaplast S.p.A. based in Suzzara MN, Via Polischi, 3. For all further details and notices relating to his/her data, the person involved should write, by registered mail, to Cisaplast S.p.A. based in Suzzara (MN), Via Polischi, 3 - or to the email address info@cisaplast.com indicating as subject matter “privacy”.
